Student Forms

For each form that requires a signature, we encourage you to print and sign if possible.  You may then take a picture or scan the form back in with your signature.  If you do not have access to a printer we would ask that in lieu of your physical signature that you send the completed/saved form back from your K-State email.  Because you have to authenticate via your K-State email that can act as your signature/approval to move forward with the form.

A/Pass/Fail Appeal Form

The purpose of this form is to appeal the addition of the A/P/F grading basis for the class(es) listed below back to a standard graded option (A, B, C, D, F).  Please list ONLY those courses you want to have the grading basis moved from A/Pass/Fail back to the Standard letter grading to the option.  

**Appeals requesting removal of the ‘A’/Pass/Fail option are only considered in cases of extenuating circumstances**

Spring 2020 Graduates:  Note that if you are considering appealing you must have your form to your academic college prior to the submission deadline of Monday, June 8, 2020.  This is to ensure that changes to your grading basis are applied prior to degree conferral.  Once your degree has been conferred (posted to your academic record) you are no longer eligible for this option.  (per the Non-Permissible Changes to the Academic Record after Degree Posting guidelines)

This form is for the Spring 2020 term only for use in response to the COVID-19 outbreak.  

The Deadline to Submit was Friday, May 8, 2020, at 5:00 pm CDT - the form is no longer available for use.  If you have submitted a form to your academic advisor/major professor by this deadline they will make sure it is funneled to the queue in the Office of the Registrar for processing.

All forms successfully submitted to the Office of the Registrar by the deadline were completed and coded in the system in time for the spring 2020 grade deadline, Tuesday, May 19th at 5:00 pm CDT
