Each form includes specific instructions for completion and submission.

PDF forms that require a Signature:
If a form requires a signature, please print and sign it if possible. You can then scan or take a photo of the signed form and email it back. If you do not have access to a printer, you may submit the completed form from your official K-State email. Because your K-State email is authenticated, it serves as your approval and signature for processing.

Important: Only one student may be submitted per form. Forms containing lists of students will be rejected.

A/Pass/Fail Opt-in	Student	Students can adjust grading basis in KSIS under “Enrollment: Edit” in Student Center. Available only for free electives. {Refer to the University Handbook, F91-93} Must be submitted before the academic calendar deadline.
Academic Forgiveness	Student	This form is used to request Academic Forgiveness and is filled out by the student and then turned in with documentation to the Academic Standards Committee or College Dean's Office of the student's Primary Major.
Academic Fresh Start	Student	This form is used to request Academic Fresh Start and is filled out by the student and then turned in with documentation to the Academic Standards Committee or College Dean's Office of the student's Primary Major.
Credit by Departmental Exam	Academic Department Use ONLY	Special Examination for Enrolled Students. For guidance on this process click here.

The Instructor will submit the form to the college dean or designee for approval The College Dean's Office will submit the completed form to the Office of the Registrar by the audit deadline

Residency

Some forms require notarization. Check for notary sections before submitting.

Application for Resident Tuition	Student	Students seeking classification as a Kansas resident are responsible for providing documentation that demonstrates they meet the criteria.
Armed Forces Residency Benefit	Student	Current member or dependent of a current member of the U.S. Armed Forces (Army, Navy, Marine Corps, Air Force, Coast Guard, Space Force, Kansas Army or Air National Guard, or any branch of the military reserves of the United States of America). Prior Service Veterans, Spouses, Dependents Kansas Board of Regents: K.A.R. 88-3-8a and the Veteran Armed Forces Services Act of 2014 (“Choice Act”)
Dependent of Divorced/Separated Parents (pdf)	Student	Dependent of a divorced/separated parent that is a resident of the State of Kansas.
Five Year Grace Verification (pdf)	Student	The 5-year grace provision of the residency laws allows a bona fide Kansas resident (did not establish residency in another state) to return to live in Kansas within five years of leaving and retain their resident status for tuition purposes.
Indigenous Peoples Residency Benefit	Student	Students who are enrolled as a member of designated nations with historical connections to Kansas are eligible for residency status at Kansas public institutions of higher education, regardless of the location of their residence. Kansas Board of Regents: K.A.R. 88-3-8a and the 2025-2026 SB125 pg. 193
HB2145 Affidavit (pdf) (Undocumented/DACA/Nonimmigrant)	Student	Some undocumented/DACA students may receive in-state tuition in accordance with KAR 76-731a.
Recruited-Transfer Employee Waiver Form	Student & Employee	The resident fee privilege is available to individuals who were recruited to move to Kansas to accept or keep full-time employment, even if they have lived in the state for less than 12 months. This benefit also extends to their spouse and dependent children.
Resident Tuition Waiver for a University Employee or a Dependent (pdf)	Student & Employee	This form allows eligible university employees or their dependents to request a waiver of non-resident tuition charges, granting them resident tuition rates in accordance with policy.

KSIS Security Access Termination Request Form	Staff/Faculty	This form is used to request the removal of a user’s access to KSIS (K-State Student Information System) when their role changes or employment ends, ensuring compliance with university security policies.
KSIS Service Indicator Request Form	Staff/Faculty	This form is used to add, update, or remove service indicators in KSIS (K-State Student Information System), which help manage special tracking/group management on a student’s record.
DARS Client Access Request Form	Staff/Faculty	For client (database) access only. Self service access is based on KSIS roles (see Role Descriptions).

Note for employees: Changes to PII (e.g., Name, Gender, SSN/ITIN) must also be reported through your HR liaison. HRIS and KSIS do not fully sync for some data types.
