1095-C tax forms now available
The State Employee Health Plan, or SEHP, has announced that 1095-C forms have been released for 2023.
Save and print your 1095-C tax form by logging in to your member portal. Form 1095-C provides information about the health coverage offered by your employer and, in some cases, about whether you enrolled in this coverage.
Once signed into the member portal, click on the "Forms" tab, then "Tax Forms," then click on the 1095-C. The form will open as a PDF for you to save or print. If you elected to have your 1095-C mailed, watch for it in your mailbox over the next several days.
