Lafene Health Center sends COVID-19 vaccine survey to faculty, staff and students

Submitted by Lafene Health Center

Kansas State University has been selected as a COVID-19 closed point vaccine distribution site. This means the university will be receiving and administering vaccines exclusively for students, faculty and staff based on the state and local priority groups and vaccine availability

K-State has not received vaccines yet or been given a vaccine delivery date. The university is actively preparing for distribution so that vaccines can be administered as soon as possible when they are available.

All current faculty, staff and students are scheduled to receive an email Monday, Feb. 1, from Lafene Health Center. This includes all employees and students on the Manhattan, Polytechnic and Olathe campuses; K-State Research and Extension sites; the K-State Alumni Association; K-State Athletics; and the KSU Foundation.

The email from Lafene Health Center will have the subject line: Important Patient Survey. The email will include a link with directions for completing a survey about the COVID-19 vaccine. Individuals will be asked to go to the myLafene+ patient portal, sign in and go to Survey Forms. Individuals who do not receive the email should contact Lafene Health Center at lafene@k-state.edu. Indicate your Wildcat ID and eID with the subject line: COVID Vaccination Request.

Completing the survey will assist K-State and local health officials in identifying how many people are in each priority group and will help communicate with individuals once they are eligible to receive a COVID-19 vaccine. The university has established a comprehensive plan based on state and local priority groups. For more information on individuals identified by the university for the phase two distribution, please see the K-State COVID-19 vaccine information website

Individuals are encouraged to complete the COVID-19 vaccine survey with Lafene Health Center and through their resident county as available. Individuals should complete the vaccination sequence through the organization that contacts them first.

The Kansas Department of Health and Environment is allocating the vaccine in the state of Kansas. Please note that the university and county health department vaccine phases are aligned. The university must follow the phase timeline for the county.

More information about the COVID-19 vaccine and a list of frequently asked questions can be found on the K-State COVID-19 vaccine information website.
