ADHD Evaluation and Testing
ADHD evaluation and Testing within Lafene Health Center and/or Lafene CAPS requires multiple appointments.
- The Case Manager will contact you via phone or through secure messaging and review the process, including the cost of testing. Currently, the cost is $416 if performed by Lafene Health Center. This charge may be billed to your health insurance or you may request self-pay option. The cost for computerized testing performed at Lafene CAPS cannot be filed to insurance and only has a self-pay option.
- After you confirm ADHD testing is desired, an appointment will be made with a mental health provider to go over symptoms, relationships, educational history, substance use, family history, etc. A waiver must be signed at this appointment and the test will be administered.
- A return appointment will be made with the same provider where you will sign a waiver, and review the results/report of the testing and recommendations. If the plan of care includes medication, you will follow up with a prescribing provider to continue care/medications.
If there are no appointments available for testing at LHC or CAPS, alternative options to complete testing outside of LHC include:

If ADHD testing is completed outside of Lafene and you want to continue your care here, then records (evaluation and testing) will need to be sent to us (FAX: 855-618-0188) for review by our psychiatric nurse practitioner.
