Changes to the Hazardous Waste Pickup Request Form
The Department of Environmental Health and Safety has made the following changes to the Hazardous Waste Pickup Request Form:
1. The name has changed to Waste Pickup Request Form.
2. You can now request a pickup of more than one container(s) on the form.
3. We've added one acknowledgement that must be made to make the form usable as a compliance document. Accurate and complete information is very important.
4. You will now be required to select each item in the certification section.
In one part of this section you will be asked to select that "I and all staff in this lab/shop have taken Hazardous Waste Awareness Training." This training is required annually. If anyone needs training please check the schedule.
