2. Complete and sign the form.
You must indicate the source of financial support. Sponsors must sign the financial affidavit; electronic signatures are not accepted. You may sign the form if you are self-funded. This documentation is required in order to issue an I-20 or DS-2019 and has no bearing on the admission decision. You MUST indicate on the form if you are bringing dependents.
3. Scan and Upload
Scan the signed document and upload a PDF to your application status at: https://gradapply.ksu.edu/apply/status (select your application, scroll down and choose Upload Materials).
4. Upload Proof of funds
Upload your proof of funds document(s), such as bank statements, etc. to your status page (same as step 3). DO NOT email the form or documents.
It may take up to two weeks after we receive your documents to process and release your I-20. You will receive a notification when the I-20 has been released. Please do not send additional email requesting your I-20.
