State survey on health insurance benefits

In an effort to determine interest in additional benefits, the State Employee Health Plan would like your feedback on what voluntary benefits are of interest to you. Voluntary benefits are products that an employee purchases for additional benefits. The plan is asking for your input on a few potential voluntary benefits plans to evaluate the level of interest. The plan wants your input on accident, cancer, hospital indemnity/intensive care plan and critical illness plan. The survey contains six questions and takes approximately five minutes to complete. The survey is open for use now and will close at 5 p.m. Feb. 10.
