2008-2009 Professional Judgment Appeal Form
This form may be submitted if a significant change has occurred to the student's and/or parent's financial situation from the previous year. After a student submits this form and the required documentation, the Office of Student Financial Assistance will determine if an adjustment to the student's aid eligibility can be made. 2008-2009 Childcare Cost Sheet
This sheet allows an adjustment to the student's cost of attendance by informing the Office of Student Financial Assistance of the child and/or children for which the student pays childcare. Please submit this sheet and any additional documentation to the address on the sheet.
2008-2009 K-State Spouse/Dependent Grant application
To be considered for the K-State Spouse/Dependent Grant for the 2008-2009 academic year, applicants must complete and submit this form to the Office of Student Financial Assistance.
Electronic Funds Transfer Form
This form provides the option of electronically transferring excess financial assistance and/or tuition refunds to a U.S. bank checking account of your choice. Completing this short EFT form saves you from standing in line or waiting for a check to be mailed to you. The funds are sent through the ACH banking system on Wednesday afternoons.
Satisfactory Academic Progress Appeal Form
This form is necessary for students who are on financial assistance exclusion. See the requirements for satisfactory academic progress. Please submit this form and any additional required documentation to the address on the form.
Veterans Information Sheet
This sheet is for veterans of the U.S. military to complete and submit. Please submit this sheet and any additional required documentation to the address on the sheet.
Computer Cost Sheet
This sheet is for students who wish to increase their cost of attendance up to $1,500 to allow for financial assistance to cover the cost of a computer. Students must contact the Office of Student Financial Assistance to make an appointment with his or her financial assistance advisor to complete this sheet.
