Mandatory Reporting to the AV and IACUC

1. Reference: Title 9, Code of Federal Regulations, Subchapter A – Animal Welfare, 2.33(3), Guide for the Care and Use of Laboratory Animals: Eighth Edition. 2011: p. 28, Guide for the Care and Use of Agricultural Animals in Research and Teaching, 4th ed., 2020: p. 10.

2. Purpose/Scope: To provide information to principal investigators about mandatory reporting to the AV and IACUC.

2.1. Reporting of unexpected outcomes or adverse events: The Attending Veterinarian and IACUC must be notified if there is an unanticipated adverse event during the execution of an IACUC approved activity. Examples of adverse events/unexpected outcomes that should be reported to the Attending Veterinarian and IACUC include:

• Higher morbidity than expected
• Higher mortality than expected
• Development of a new disease or condition that affects the health and/or welfare of the animals

2.2. Reporting of problems with animal health, behavior, and well-being: Any incident that can affect animal health, behavior, and well-being must be effectively communicated to the Attending Veterinarian. Examples of animal health events that should be reported to the Attending Veterinarian and IACUC include:

• Animal injury, illness, or deaths resulting from HVAC issues, electrical problems, or facility problems
• Serious animal injuries or illness in herds, flocks, or individual animals, even if they are under treatment through a veterinary service
• Accidents resulting in illness, injury, or death to an animal
• Animals found dead unexpectedly

2.3. Available methods of reporting to the Attending Veterinarian:

• Contact numbers are also posted throughout facilities on the Disaster Preparedness Roster.
• Phone call or email to University Research Compliance Office (URCO): (785) 532-3224 or comply@ksu.edu
• An initial notification should be made promptly within 24 hours (same day) of the event (within 48 hours at the latest). Then additional/follow-up details/findings can be reported as they become available.
