Click on the "Subscribers Corner" link and sign in (you will have to use your e-mail address and your LISTSERV password - follow the link to get a new one if you have never set one or don't remember it). The table will show all your lists and clicking on any list name will show your options for that list.

Note the button that says "Leave " is how you unsubscribe from the list - not how you leave this page! Your page may look different if the list owner has changed it - contact them if you have any questions.
