Employees with an existing checking or savings account may make additions or changes to their direct deposit by using Employee Self Service OR by completing an Authorization for Direct Deposit of Employee Pay Form (PER-58). The Direct Deposit form should be completed with your department HR Liaison and submitted to or sent via campus mail to Human Resources at 111 Dykstra Hall, 1628 Claflin Rd., Manhattan, KS 66506.
