Late enrollment fee request waiver

Late enrollment fees are assessed when a student's initial enrollment for the semester occurs on or after the first day of classes.

This form is intended only for submission by academic departments or dean's offices on behalf of a student when late enrollment results from an administrative delay. Requests submitted directly by students will not be accepted or considered. Late enrollment fees must be petitioned within 90 days of their assessment.
