COVID-19 vaccine requirement

Ongoing COVID-19 vaccine clinic

Lafene Health Center continues to offer COVID-19 vaccine opportunities for students, faculty and staff. Lafene is offering the one-dose Johnson & Johnson and the two-dose Moderna vaccines as available.

COVID-19 vaccines are available from Monday through Friday. Appointments are required and can be made on the myLafene+ patient portal or by calling Lafene Health Center at 785-532-6544.

COVID-19 vaccination documentation

Students, faculty and staff can voluntarily upload their COVID-19 vaccine cards to the myLafene+ patient portal. By uploading the cards, K-Staters can help Lafene Health Center continue to track vaccination rates at the university. Providing proof of vaccination is not a requirement at this time and doing so is voluntary.

Follow these steps to voluntarily upload your COVID-19 vaccine card.

  1. Log in to the myLafene+ patient portal.
  2. Navigate to the Medical Clearances section at the top of the left menu.
  3. Click on the green Update button in the COVID Vaccine section at the bottom of the page.
  4. Upload an electronic copy of your COVID-19 vaccination card. The electronic copy can be in JPG, JPEG, PNG, PDF or GIF format. You can take a photo of your vaccine card or scan an electronic version of the physical card.
