What to Disclose in Cayuse OI
The questions you see in your Annual Disclosure are based on your answers to the first two pages of questions in Cayuse OI. Below is an in-depth explanation of all of the questions in Cayuse OI.
Introductory and Background Questions
These questions are on the first two pages of Cayuse OI. Because the Annual Disclosure is now a smartform, your answers to all of these questions will determine the substantive questions that appear in your disclosure form.
Explanation:
This question helps to provide the correct set of questions for disclosers. Staff and part-time or non-tenure track faculty who are not involved in research only fall under the Kansas Board of Regents COI policy and will see a more limited set of questions. Full-time faculty and researchers will see additional questions that allow this disclosure to comply with all federal research-based COI rules and regulations.
Select the option that best describes your role at K-State. If you are a temporary faculty member of any type, select the option "Part-time, non tenure track, or adjunct faculty member"
Explanation: If you selected either part-time/non tenure-track faculty or staff you will see the option to declare whether you conduct research at K-State. If you do not conduct research at the University, you will see the standard set of questions. However, if you indicated that you are involved in University research you will the additional options that allow this disclosure to be compliant with federal COI policies and regulations.
Explanation: You can use the Annual Disclosure form to request prior approval for consulting or outside employment at any time including during the annual update. You do not need to submit a separate form for prior-approval requests if you are already submitting an Annual Disclosure for other purposes.
However, if you only need to request prior approval, you should select yes to this question and completely fill out the rest of the form with all relevant activities over the previous 12 months. Cayuse OI only carries over data from the previously submitted disclosure. If you already completed information about your outside activities and you want that to carry over to next year's disclosure, you must add the activities to all submissions including ad hoc disclosures and prior approval requests.
Explanation: Answering "no" this question will not allow you to disclose any outside activities or financial interests. Generally, if you conducted any work outside of the University, received $5,000 or more in compensation, or held equity you should be answering yes to this question.
Explanation: Each selection in this question will generate a corresponding section of questions that can be found in the left-hand menu. You should select all that apply even if two categories apply to the same entity. ALL activity with entities outside of the United States should be reported regardless of if that activity fits into a different category. There is "catch-all" option to report these types of activities.
You can return to this question at any time prior to your submission of the form to select or deselect options for which you need to disclose.
