Step 1: Final Exam Ballot

Complete and submit the Approval form at least two weeks prior to the final examination. Follow these steps listed below:

1. Click on the appropriate form.
   • Doctoral Students
   • Masters Students

2. Complete the form online.
   These are "fillable" forms, so you can enter most of the required data online.

3. Print the form.

4. Obtain signatures.
   Your major professor, committee members, outside chairperson if a doctoral student, and department head or graduate program director will need to sign and date the form.

5. Submit the form.
   Turn in the completed form, along with a copy of your ETDR title page and abstract, to the Graduate School, Room 103 Fairchild Hall. You can deliver the form in person or mail it to this address:
