Step 2: ETDR Ballot

  1. Obtain Your Professors Signature
    The ETDR Ballot is sent as an e-mail attachment to your major professor prior to your final examination. Your major professor will need to print, the ETDR ballot and obtain the supervisory committee's signatures on the form.

  2. Submit the Form
    You or your major professor will need to turn in the completed form to the Graduate School, Room 103 Fairchild Hall, prior to your ETDR submission. You can deliver the form in person or mail it to this address:
