Adobe
Acrobat Reader (shareware) must be installed on your
computer to print
or
complete the PDF
forms. The complete Adobe Acrobat software (not
shareware) is required to save a completed PDF form.
Rich Text Format (RTF) forms may be used by any word
processor (Word,
Works,
WordPerfect, etc.) and do not require special software.
Form directions and/or additional information are
provided in the Instructions or Information Column or
within the form.

Complete all of the requested information on the Request for
Certified
Names PER-5.
Authorized department personnel sign the block
Department
Head's Signature. If the department head digital signature on that
computer has not been
set up before, take the steps below to set up the signatures.
Signing the document requires that the form is saved. Use position
number for the file name.
Note the location
and attach the form by email to employment@ksu.edu and copy (CC) the
department head.

Add Digital ID shows up the first time you sign Choose "A new digital ID I want to create now"
Choose "New PKCS#13 digital ID file"
Enter identity information
Choose password and confirm
Enter password and select sign
Signature prompts to save the document. Choose location and name. For
the PER-5, use position number.
