Complete an online health assessment by Sept. 30 and earn a gift card
Time is running out. If you have not taken the online health assessment in 2011, you must complete it by Sept 30 to earn a $50 gift card.
Benefits eligible state employees who are enrolled in the state employee health plan or who have waived coverage in the plan are eligible. Also, spouses and dependents 18 years or older who are enrolled in the medical portion of the state employee health plan are eligible. This gift card program will not be offered next year.
