Accident Report (Workers' Compensation)
Work-related accidents and injuries should be reported through the Electronic Incident Reporting system. Paper forms will no longer be accepted to report incidents.

Incidents related to individuals who are not university employees can also be reported through the Electronic Incident Reporting system. Questions may be directed to the Department of Environmental Health and Safety for incidents not related to employment.
