On this page you will find the forms for the Voluntary Separation Incentive Program (VSIP). Please take the time to review these forms in their entirety before signing and submitting them. We strongly encourage all who are considering this program to review these documents with the tax, financial or other advisors of their choosing.

VSIP Notice of Interest

The Voluntary Separation Incentive Program (VSIP) Notice of Interest form formalizes an employee's interest in the Voluntary Separation Incentive Program. This form marks the first step in the VSIP process and should be submitted during the application period which begins on December 19, 2022 and ends on January 27, 2023. Employees who are considering applying for the program are encouraged to take the time during the application period to discuss this program with their family and personal financial and tax advisors as K-State cannot provide financial, tax, or legal advice related to this decision. This should be done prior, if the application is approved, to signing the Notice of Retirement: Acceptance of Offer to Participate form.

This form should be submitted directly to your dean no later than 5:00 p.m. CT on Friday January 27, 2023.

VSIP Withdrawal Form

The Voluntary Separation Incentive Program (VSIP) Withdrawal form is submitted when an employee no longer wishes to be considered for the VSIP for retirement. Through this form, the employee withdraws their VSIP application from consideration provided the Notice of Retirement has not already been executed. This form would represent the final opportunity to withdraw from consideration and may be submitted any time during the application and notification period (December 19, 2022-February 24, 2023). 

The VSIP Withdrawal form should be submited to benefits@ksu.edu. Please note, once a Notice of Retirement document has been executed, an employee must retire at the separation date. 

VSIP Notice of Retirement

Employees complet the Voluntary Separation Incentive Program (VSIP) Notice of Retirement form after being notified by the office of the provost that they have been approved to participate in the Voluntary Separation Incentive Program. This form represents official acceptance of the program and official notice or retirement. Once completed, the notice of retirement will be treated as final and irrevocable. Employees acknowledge that they have accepted the terms of the VSIP and are committed to retire on May 13, 2023 for 9-month or June 10, 2023 for 12-month faculty appointments. The deadline for submitting this form is February 24, 2023.

This form should be submitted to benefits@ksu.edu.

SAMPLE VSIP Separation Agreement with Waiver and Release

The VSIP Agreement with Waiver and Release is the final step in the VSIP process and is used to formalize the Voluntary Separation Incentive Program for retirement between the employee and Kansas State University. The final version of this form ready for signature, will be provided to approved participants no later than April 14, 2023.  The deadline to execute and submit this form is May 1, 2023.  This is the final step in the Voluntary Separation incentive Program.

The VSIP Separation Agreement is not due until May 1, 2023. Please do not sign and submit this document to your dean prior to April 14, 2023.
