Production Forms

Following are some helpful forms we use for instructional capture, lectures, and other events to ensure we arrive on time, with the right equipment and personnel ready to capture your event. All forms are electronic, but may be printed for written entry and returned by facsimile. Please download, open the form with Adobe Acrobat or Reader (doesn't work in Preview or other non-Adobe PDF software) and complete. To submit the form electronically, click the "Submit" button (on those forms so equipped), or save your completed form under a new file name, and attach it to an email to ves@k-state.edu with the form name, date, time and venue in the subject field of the email. When received, we'll review the form, confirm receipt, and contact you if we have questions.

Event Form

The best way to ensure your event is on our production calendar. If you're unsure of the appropriate response, please mark the field with an "?" or leave it blank - we'll contact you to clarify upon receipt.

Full Use Release Form

For speakers/lecturers visiting Kansas State University (non-faculty or staff), we require a signed release allowing the use of their image and presentation contents prior to the event capture. This ensures we have rights allowing posting online and rebroadcast (on K-State TV/Channel 8/YouTube) as needed. This form provides for "full use," allowing the university to utilize the presenter's image in promotional materials for the university. When possible, this is the preferred release as it allows units and departments to show speakers in promotion materials.

Educational Use Release Form

Fillable PDF form - for speakers/lecturers visiting Kansas State University (non-faculty or staff), we require a signed release allowing the use of their image and presentation contents prior to the event capture. This ensures we have rights allowing posting online and rebroadcast (on K-State TV/Channel 8/KSOL) as needed. This form provides for educational use only, and precludes use of the presenter's image or materials in promotional materials for the university. This form should be utilized if the speaker does not what their image utilized for any purpose but educational use.
