A standard heading for forms has been developed to make K-State forms recognizable and consistent.
The K-State logo is placed at the top righthand corner of all forms with a solid bar. The form name and office or unit identification are printed above the bar.

Text below the bar may be set up according to your needs and preferences. Shown here are examples of forms that incorporate the standard heading at the top of the page.

Two other examples of forms with standard heading formats are routing slips and fax sheets. You may choose one of the three logos for your heading, and place elements or names you would like on the routing slip. You may also customize copy below the bar on the fax sheet.



Headings
Headings above the bar should be set in Futura condensed if set at K-State Printing Services or if you have that typeface available on your system. Otherwise the recommended typeface is Helvetica compressed or the best sans serif typeface match.

The form title should be set in 33/35 Futura bold or 22/18 if the title is long.

The address should be set in 8/10 Futura regular with the office name in 9/10 Futura bold.

Routing slips and fax sheets


Recommended type specifications are:
Routing slips: headings are 22/18 Futura compressed, with the office name in 9/10 Futura. Text below the bar is to be set in 8/10 Helvetica.

Fax sheets: headings are set in 22/18 Futura compressed, with text below the rule set in 10/15 Helvetica bold.
