HealthQuest deadlines approaching
July 31 is the deadline to qualify for a $480 state employee health insurance premium incentive discount for 2015.
Completing the health assessment questionnaire — worth 10 credits — and earning an additional 20 credits — for a total of 30 credits — by July 31 is how employees qualify for a $480 health insurance premium incentive discount for plan year 2015. Employees can confirm their discount status by logging into their HealthQuest account, and clicking on the "My Printable Credits Summary" link from the left menu. Employees who requested an at-home screening kit to obtain their biometric values should return their sample in the provided self-addressed, stamped envelope no later than July 1.
Employees are notified by email once their results have been loaded into their online health assessment questionnaire — allow 15 business days. They will need to log in to their account to finish the additional questions by the July 31 program deadline to earn credits toward the 2015 premium incentive discount. Employees who requested a paper health assessment form must complete and return the questionnaire with a postmarked date no later than July 1 to earn credits toward the 2015 health insurance premium incentive discount.
