How to earn credits for health coaching, condition management or tobacco cessation

By Division of Human Resources

Employees in the State Employee Health Plan currently enrolled or planning to enroll in any of the ongoing health coaching, condition management or tobacco cessation programs that take place over several weeks or months should strive to complete the specific program requirements by July 31 in order to receive their 10 credits that will count toward the 2015 premium incentive discount.

For health coaching participants, a total of eight two-way interactions with your health coach plus completion of a survey within six months of enrollment is required to earn credits. For condition management, enrolled participants must complete at least three calls with a nurse to earn credits. tobacco cessation participants will need to complete five calls with a quit coach to earn credits.

To enroll in one of these programs or to confirm your status toward completion, please call 1-888-275-1205 to speak with a coach/nurse in your specific program. For more details, go to State of Kansas Health Quest Credit Options.
