This form may be submitted if a significant change has occurred to the student's and/or parent's financial situation from the previous year. After a student submits this form and the required documentation, the Office of Student Financial Assistance will determine if an adjustment to the student's aid eligibility can be made. 2011-2012 Childcare Cost Sheet
This sheet allows an adjustment to the student's cost of attendance by informing the Office of Student Financial Assistance of the child and/or children for which the student pays childcare. Please submit this sheet and any additional documentation to the address on the sheet.
2011-2012 Computer Cost Sheet
This sheet allows an adjustment to the student's cost of attendance toward the purchase of a computer. Students are encouraged to visit with his/her financial aid advisor to determine what impact the change will have on their aid eligibility.

Parental Electronic Funds Transfer Application
This application provides the option to parents who borrow the Parent PLUS loan of electronically transferring excess financial aid and/or tuition refunds to a U.S. bank checking account of their choice. Completing this short EFT form saves parents from waiting for a check to be mailed to them.
