IBC Research Registration Document

Instructional/Teaching Registration Document
For Teaching and Instructional Activities Involving Recombinant DNA,
Infectious Agents, and/or Tumorigenic Material

NOTE: We frequently update the online application, so please remember to use the latest version when submitting an application.

The application is a word processing document available in Word or WordPerfect format. At the prompt, choose "save it to disk" to complete the form in either word processing program. The shaded areas are text boxes designed to accommodate responses within the body of the application. As you type your answers, the text boxes will expand as needed. After completion, print the form and send the original to 203 Fairchild Hall. If you have any problems or questions regarding this process, please contact our office for assistance.

Containment Level Flow Chart - Flow chart/information to assist assigning risk level and section of the NIH guidelines in the IBC registration document. This flow chart is available in the following two formats:

  1. As a Training Module.
  2. As an HTML Web Page.

If you are unable to open any of the above forms, we also have the application available in Plain Text format. Please use this only if you are unable to open any of the above forms as it does not have any of the pre-defined formatting and will take longer to process.

Modifications

For minor modifications: Submit a letter (preferably on an official letterhead) explaining the modifications in detail and to what section of the IBC protocol they apply to. Also, please state in your letter that nothing else has changed with your protocol with the exception of these minor modifcations.

For major modifications: Please resubmit your original approved IBC protocol with the changes integrated into the appropriate protocol sections. It is required that the changes be denoted somehow from the original protocol text (bold face, different font or size) so that they are easily discernable from the original text.
