Animal Monitoring and Observation - Forms


Recognition is key to pain and distress management since signs may be difficult to detect or evaluate. The IACUC will require investigators to consider use of an Animal Monitoring Plan (AMP) and Animal Observation Record (AOR). The AMP along with the AOR detail how animals will be monitored and what actions will be taken in order to minimize pain or distress associated with the research project. Investigators must indicate if an AMP and AOR are required on the IACUC protocol application (section VI.H.1). In addition to assisting with day-to-day monitoring, the forms will provide ready documentation of active pain and distress management for regulatory and accrediting agencies such as the USDA, OLAW, and AAALAC. The AMP is a part of the protocol application and should be completed within the body of the application. Use the forms below if you require additional sheets.
