IACUC Application Forms

    The IACUC application is a word processing document available in Word or WordPerfect format. To fill out an application, choose your format, and click on the appropriate link. At the prompt, choose "save it to disk" to complete the form in a word processing program saved on your hard drive. The shaded areas in the proposal are text boxes designed to accommodate responses within the body of the application. As you type your answers, the text boxes will expand as needed. When completed, it is advisable to save a copy of the completed proposal, as it is possible that the IACUC may ask you to revise it following review.

      Renewing An Approved IACUC Protocol

      After 3 years, all IACUC protocols will expire. To renew, a new IACUC proposal must be sumitted even if mothing has changed regarding the project. The URCO staff will receive, process, and assign a new number to this proposal. In addition, the proposal will be reviewed by the IACUC or one of its committee members. All work on expried approved IACUC protocols must stop until approval for the new proposal is received. Thus, it is in the best interest of the Principal Investigator to allow ample time for a new proposal to be submitted, processed, and approved so as not to incur any break in research. The URCO staff will send out reminder letters informing of the need for protocol renewal 3, 2, and 1 month prior to actual protocol expiration date.

    The IACUC has approved the type protocol below for simple teaching activities. For clarification on when and how to use this addendum, please contact the Research Compliance Office at 532-3224.

    Modifications

      Modifications to Approved IACUC Activities (PDF format)

      For minor modifications: Submit a letter (preferably on an official letterhead) explaining the modifications in detail and to what section of the protocol they apply to. Also, please state in your letter that nothing else has changed with your protocol with the exception of these minor modifcations.

      For major modifications: Please resubmit your original approved IACUC protocol with the changes integrated into the appropriate protocol sections. It is required that the changes be denoted somehow from the original protocol text (bold face, different font or size) so that they are easily discernable from the original text.

    Food Animal Feeding or Production Activities “Blanket Protocol” Modification Request

      Modification Request for Animal Use in Non-Invasive, Non-Painful Food Animal Feeding or Production Activities (Requires an approved Blanket Protocol)

      The IACUC must review and approve animal care and use activities prior to initiation of the activity. Food animal feeding or production trials frequently involve large numbers of animals using standardized procedures with changes only in ingredients or additives. It is difficult to predict all of the potential variations of a feeding trial that may be performed.

      Consequently, in order to facilitate the submittal and review of food animal feeding trials involving large numbers of animals, investigators can:

      • Submit a request for a “blanket protocol” that describes the overall activity in broad terms. The blanket protocol, submitted on the standard application form will be reviewed and approved by the IACUC.
      • Then when the specific ingredients, additives, or variables are determined, a modification request on the simplified form can be submitted. Because the blanket protocol has been approved, the modification request will be considered a “minor modification” and can be quickly approved by Chair of the IACUC.

    Mandatory Training

    The Animal Welfare Act and the PHS Policy require that personnel are appropriately trained in animal care and use, and that the training is documented. The P.I. is responsible for ensuring that all study personnel have completed appropriate animal care and use training. Prior to final approval of an animal care and use protocol, the IACUC requires completion of required online training modules for all personnel listed as participating in the proposed animal care and use activity. Consequently, in order to avoid delays in final approval, please advise your technicians and/or collaborators that they must complete the required online training modules in a timely manner. The URCO will maintain documentation of completion of the online training in a database, and will cross-reference training for designated participants prior to final protocol approval. Any person listed in Section VII. Investigator and Technician Qualifications /Training of the IACUC Application for Approval Form, will be expected to complete required training modules.

    Upon receipt of your proposal, the URCO will contact you regarding the training modules that will be mandatory before final IACUC approval of your proposed activity. Training Module 1, Introduction and Overview of Research Involving Animals is mandatory for all persons involved in research, training or testing with animals at KSU. Additional training modules are targeted for species or procedures specific to your proposed activity.

    The required online modules are a minimum training requirement. Other more targeted and specific procedural training may also be necessary for a proposed activity, e.g., vessel cannulations, biopsies, etc. Maintaining documentation for training beyond the minimum online requirement is the responsibility of the PI.
