| Consent to Disclose Educational Records | Submit this form to the Office of the Registrar to provide consent for your educational records to be disclosed to a third party. |
| Enrollment Verification Request Form | Please see the Degree and Enrollment Verifications page for information regarding the enrollment verification request form. |
| Information Change Form | Submit this form to the Office of the Registrar in order to change your information on iSIS such as changing marital status from “single” to “married”, your name, your birth date, or your address (only if you are a past student). |
