EDUCATIONAL MODULES
Please note that the documents linked below are partial and do not include the entire text of the modules. To request the complete text of these modules please send an email to gerontology@ksu.edu with the following information:

Name & Title of Requestor
Name of HealthCare Facility & number of residents served
Complete mailing address including zip code
Telephone number
Email address

Modules will be sent free of charge to HealthCare Facilities in Kansas. (Duplicate copies for Kansas facilities will require a small fee to cover the additional costs of shipping). Requests outside Kansas will be charged a modest fee to cover the costs of materials, postage and handling.

Modules will be sent on a CD, saved in pdf format.  The files can be opened with Adobe Reader software, available as a free download from www.adobe.com/products/acrobat/readstep2.html.   More information can be found in the "About this module" section when clicking on the links below.

At the bottom of this page you will find a link to sample activities which may be appropriate for staff training or in-service presentations.

The Family Resource Guide was designed to accompany the Family & Community Module.  It is in a printable format appropriate for 8 1/2 x 14 paper. It should be printed or copied double sided, in the order in which it is listed in the file. Blank pages have been added to the file, which are necessary for the booklet to print accurately and in the correct order. The booklet is available here in Adobe Acrobat format.
