Affidavit of Support Graduate Admissions

This form is extremely important. Documentation of financial support is required by the US Citizenship and Immigration Service and allows KSU to issue the papers you use for obtaining a student visa.

You must provide proof of financial support for the period you plan to come to KSU under the exchange program. Under the provisions of the agreement on which you are coming to KSU, you pay tuition and fees at resident rates. This substantially reduces the total costs for which you would otherwise be required to provide proof of support as indicated on the reverse of this form. You must have, however, sufficient resources to cover the costs of resident tuition, fees, room, board, books, insurance and other personal expenses. The amounts the KSU Graduate School calculates to cover these expenses are:

$8,138 for one semester
$16,275 for two semesters
$4,420 for the summer session
$20,695 for 12 months (use this amount if your applying as a degree-seeking student)

Section I: Declaration

Either yourself or your sponsor must fill in this section.

Calculate the amount for which you must provide proof of financial support from the cost information given above and for dependents, if applicable. Cross out the $29,850 and write the calculated amount above the crossed out amount on the affidavit form.

Section II: Sponsor's Declaration

Provide all requested information.

Section III: Bank Declaration

Have a bank or other such financial institution complete this with proper signature and seals/stamps. Alternately, the same information may be written and signed/sealed on the institution's stationery.
