Student Biographical Information Form

The information you provide may be used for immediate or future news articles about you. The more information you provide us, the better we will be able to write a comprehensive article now or in the future. Your electronic signature grants us permission to use information you provide.

Additionally: If you have signed a form in the registars office to prevent your information from being released, media relations cannot publicize your achievements without an additional form. If this is your case you may visit media relations at 9 Anderson Hall and fillout a release form that will apply for one specific news release.
