Vaccination Requirements

Kansas State University vaccination requirements must be completed prior to the start of the second semester classes. Failure to complete the applicable requirement(s) may result in a hold placed on your student account, prohibiting enrollment in classes, dropping classes and access to transcripts.

Measles, Mumps, Rubella

Two (2) doses of MMR or a lab report confirming immunity.

The requirement applies to:

DOMESTIC STUDENTS – You must complete this requirement if you meet all of the following criteria:

  • You are new to Kansas State University – have never before attended classes at Kansas State.
  • You will be attending the Manhattan campus.
  • You will be taking at least one (1) hour of class in a face-to-face setting.

This requirement is a Kansas State University policy which includes the following vaccines given in accordance with the guidelines set forth by the Centers for Disease Control and Prevention (CDC).

INTERNATIONAL STUDENTS – You must complete this requirement if you meet all of the following criteria:

  • You are an international student with an F-1 or J-1 student visa.
  • You are new to Kansas State University – have never before attended classes at Kansas State.

This requirement is a Kansas State University policy which includes the following vaccines given in accordance with the guidelines set forth by the Centers for Disease Control and Prevention (CDC).

HOUSING STUDENTS (Domestic AND International) – You must complete this requirement if you meet all of the following criteria:

  • You are a Kansas State University student.
  • You are new to University Housing – have never before lived in University Housing.

This requirement is a Kansas Board of Regents policy (Chapter II, B. 4. b.) which includes the following vaccines given in accordance with the guidelines set forth by the Centers for Disease Control and Prevention (CDC).

Meningococcal conjugate (ACWY)

One (1) dose received since age 16. 

To complete your vaccination requirement(s) you must both ENTER AND UPLOAD your vaccination(s) into Lafene Health Center’s secure portal:

        Log in to myLafene+ with your eID and password.

  • Click on the Forms tab.
  • Immunization History Form.
  • Complete the electronic form by transferring the dates from the paper form completed by your licensed medical provider to the electronic form.
  • Click on the ADD IMMUNIZATION RECORD to upload your official records.
  • Click on the SUBMIT FINAL button.

To complete this vaccination requirement with a waiver, click on Housing Meningococcal Waiver, print, complete the waiver and return it to Lafene Health Center by the secure message option as described below or by fax 785.532.3425 or by mail to Lafene Health Center, 1105 Sunset Ave, Manhattan KS 66502.

HOUSING MENINGOCOCCAL WAIVER 

**If you encounter technical difficulties, use the secure messaging option by clicking on the Messages tab. Select “I want to send a message to the Immunization Nurse”. Enter Immunization Record in the SUBJECT: area and any other comments you may have in the compose message box. Once completed, press SEND.  Vaccination records submitted by this method may take up to fifteen (15) working days to be verified and posted to the student’s account.

PROCESSING OF SUBMISSIONS

All submissions sent through the secure portal are processed within ten (10) business days of receipt. The entered dates of immunizations and the uploaded immunization records will be reviewed for completeness and compliance. If there is an issue with the information submitted, Lafene Health Center will send you a secure message through the secure patient portal explaining the issue and how to resolve it.

A student will be considered non-compliant if:

  • Vaccination dates are not entered into the secure portal’s Immunization History Form or the appropriate documents are not uploaded for verification.
  • Incomplete documentation is received.
  • The documentation is illegible.
  • Vaccinations are contrary to Kansas State University requirements based on CDC requirements.

In the event of non-compliance, the student will be notified via the secure patient portal and instructed how to become compliant.  This may include:

  • Repeating the entry of vaccinations through the secure patient portal.
  • Uploading documentation.
  • Proof of immunity through a titer lab test.
  • Repeat of the vaccination.

Exemptions

Medical

If you have a medical condition which prohibits you from completing one (1) or more of the requirements, you may submit an EXEMPTION request. Medical exemptions must include a statement completed by your licensed medical provider (MD, DO,APRN, PA, RN) and submitted using the secure messaging option as explained above.

Good Cause/Religious/Philosophical/Moral Conviction

If you object to one (1) or more of the requirements, based upon good cause, religious, philosophical, or moral convictions, you may submit an EXEMPTION request. The exemption must state the reason and be notarized by a Notary Public. 

All EXEMPTION Requests must be discussed in person with the Medical Director or his/her designee of Lafene Health Center. Please call the Health Center to make an appointment with the Medical Director.
