The Medical (Health) history form must be completed and signed by the student prior to the student receiving services. Students under the age of 18 must have parental permission to obtain health care at Lafene Health Center. This may be accomplished by signing the health history form. This form is available at New Student Orientation, at Lafene's front desk or can be mailed to you for your convenience. It is often easier (and highly recommended) to have this form on file at Lafene before the student is injured or ill.
To obtain the form by mail, send an e-mail to lafene@k-state.edu. Type Medical History Form Request in the subject area and be sure to include name and mailing address in the body of the message. Return the completed form to Lafene Health Center: Attention - H.I.M. (Health Information Management department).
