Contact the University Research Compliance Office (URCO) at 785-532-3224 and describe your survey.
If URCO determines that your survey falls into the category 'Research', per the federal definition, you must use the "IRB Application". If your survey is classified as 'Non-Research' you must use the "Class Project Form". Both forms can be found at http://www.k-state.edu/research/comply/irb/forms/index.htm.
Submit the appropriate completed form, along with a copy of your survey questions, to Heath Ritter at 203 Fairchild Hall. If your survey is approved, you will receive a "Letter of Approval".
After all approvals have been obtained, you may choose to have your respondents populated by K-State. You will need to complete the Request for populating an Axio Survey form found at https://www.k-state.edu/its/forms/surveypopulation.html.
