Telephone Work Orders

The following are the types of Telephone work orders that can be submitted:

New Long Distance Authorization Code

  1. Go to https://kstate.service-now.com/.
  2. Under Self-Service, click Service Catalog.

  3. In the Telecom Work Orders box, click Telephone Work Orders.

  4. Click New Long Distance Authorization Code.
  5. In the On Site Contact (eID) box, type the eID of the person who is the on site contact or click the magnifiying glass to search for the contact, and then click the contact's name.

    Note: By deafult, the person filling out the request is displayed in the On Site Contact (eID) box.

  6. In the Contact Phone Number box, type in the contact phone number of the person you would like to be contacted in case there are questions about the request.
  7. In the Requested For User (eID) box, type the eID of the person you are requesting the service for or click the magnifiying glass to search for them and then click their name.

    Note: If this request is for someone who doesn't have an eID or if it is for a location that doesn't have an eID associated with it, then just put in either the on site contact or the person filling out the request. In the Notes/Comments box, describe who the request is for.

  8. In the FIS account box, type the FIS account number or click the magnifiying glass to search for the account and then click the FIS account name to select it.
  9. In the Departmental Account box, type the Departmental account or click the magnifiying glass to search for the departmental account and then click the departmental account name to select it.
  10. In the Notes/Comments box, type any desired notes.
  11. In the Order this item section, click Add to Cart button and then click the Continue Shopping button. When you are finished requesting items, click the Proceed to Checkout button.

    Note: An email will be sent to you confirming your order.

New Long Distane Kans-an card

  1. Go to https://kstate.service-now.com/.
  2. Under Self-Service, click Service Catalog.

  3. In the Telecom Work Orders box, click Telephone Work Orders.

  4. Click New Long Distane Kans-an card.
  5. In the On Site Contact (eID) box, type the eID of the person who is the on site contact or click the magnifiying glass to search for the contact, and then click the contact's name.

    Note: By deafult, the person filling out the request is displayed in the On Site Contact (eID) box.

  6. In the Contact Phone Number box, type in the contact phone number of the person you would like to be contacted in case there are questions about the request.
  7. In the Requested For User (eID) box, type the eID of the person you are requesting the service for or click the magnifiying glass to search for them and then click their name.

    Note: If this request is for someone who doesn't have an eID or if it is for a location that doesn't have an eID associated with it, then just put in either the on site contact or the person filling out the request. In the Notes/Comments box, describe who the request is for.

  8. In the FIS account box, type the FIS account number or click the magnifiying glass to search for the account and then click the FIS account name to select it.
  9. In the Departmental Account box, type the Departmental account or click the magnifiying glass to search for the departmental account and then click the departmental account name to select it.
  10. In the Notes/Comments box, type any desired notes.
  11. In the Order this item section, click Add to Cart button and then click the Continue Shopping button. When you are finished requesting items, click the Proceed to Checkout button.

Remove Long Distance Authorization Code

  1. Go to https://kstate.service-now.com/.
  2. Under Self-Service, click Service Catalog.

  3. In the Telecom Work Orders box, click Telephone Work Orders.

  4. Click Remove Long Distance Authorization Code.
  5. In the On Site Contact (eID) box, type the eID of the person who is the on site contact or click the magnifiying glass to search for the contact, and then click the contact's name.

    Note: By deafult, the person filling out the request is displayed in the On Site Contact (eID) box.

  6. In the Contact Phone Number box, type in the contact phone number of the person you would like to be contacted in case there are questions about the request.
  7. In the Requested For User (eID) box, type the eID of the person you are requesting the service for or click the magnifiying glass to search for them and then click their name.

  10. In the Long Distance Authorization Code box, type the Long Distance Authorization Code that was issued to you by the Telecom Office
  11. In the Notes/Comments box, type any desired notes.
  12. In the Order this item section, click Add to Cart button and then click the Continue Shopping button. When you are finished requesting items, click the Proceed to Checkout button.

    Note: An email will be sent to you confirming your order.

Remove Long Distance Kans-an access

  1. Go to https://kstate.service-now.com/.
  2. Under Self-Service, click Service Catalog.

  3. In the Telecom Work Orders box, click Telephone Work Orders.

  4. Click Remove Long Distance Kans-an access.
  5. In the On Site Contact (eID) box, type the eID of the person who is the on site contact or click the magnifiying glass to search for the contact, and then click the contact's name.

    Note: By deafult, the person filling out the request is displayed in the On Site Contact (eID) box.

  6. In the Contact Phone Number box, type in the contact phone number of the person you would like to be contacted in case there are questions about the request.
  7. In the Requested For User (eID) box, type the eID of the person you are requesting the service for or click the magnifiying glass to search for them and then click their name.

  10. In the Kans-an card number box, type the Kans-an card number that was issued to you by the Telecom Office
  11. In the Notes/Comments box, type any desired notes.
  12. In the Order this item section, click Add to Cart button and then click the Continue Shopping button. When you are finished requesting items, click the Proceed to Checkout button.

    Note: An email will be sent to you confirming your order.

New Telephone

  1. Go to https://kstate.service-now.com/.
  2. Under Self-Service, click Service Catalog.

  4. Click New Telephone.
  5. In the On Site Contact (eID) box, type the eID of the person who is the on site contact or click the magnifiying glass to search for the contact, and then click the contact's name.

  8. In the FIS account box, type the FIS account number or click the magnifiying glass to search for the account and then click the FIS account name to select it.
  9. In the Departmental Account box, type the Departmental account or click the magnifiying glass to search for the departmental account and then click the departmental account name to select it.
  10. In the Display name box, type the customer's name as they would like it to appear on the caller ID.

    Notes:

    • The display name can be user name, department, location, etc.
    • The display name should be limited to the number of charcters that can be put into a phone switch.

  11. In the Building box, type the Building name or click the magnifiying glass to search for the building and then click the building name to select it.
  12. In the Room Number box, type the Room Number.
  13. In the Phone Type section, click the type of phone you are requesting.
    • Analog - single line, desk phone
    • Analog Cordless - cordless, single line, desk phone
    • Wireless IP - multi-line phone, utilizes campus wireless IP infrastructure
    • Digital - multi-line desk phone
  14. Under Add Phone Options, select desired phone options.
  15. In the Notes/Comments box, type any desired notes.
  16. In the Order this item section, click Add to Cart button and then click the Continue Shopping button. When you are finished requesting items, click the Proceed to Checkout button.

    Note: An email will be sent to you confirming your order.

Move Telephone

  1. Go to https://kstate.service-now.com/.
  2. Under Self-Service, click Service Catalog.

  3. In the Telecom Work Orders box, click Telephone Work Orders.

  4. Click Move Telephone.
  5. In the On Site Contact (eID) box, type the eID of the person who is the on site contact or click the magnifiying glass to search for the contact, and then click the contact's name.

    Note: By deafult, the person filling out the request is displayed in the On Site Contact (eID) box.

  6. In the Contact Phone Number box, type in the contact phone number of the person you would like to be contacted in case there are questions about the request.
  7. In the Requested For User (eID) box, type the eID of the person you are requesting the service for or click the magnifiying glass to search for them and then click their name.

    Note: If this request is for someone who doesn't have an eID or if it is for a location that doesn't have an eID associated with it, then just put in either the on site contact or the person filling out the request. In the Notes/Comments box, describe who the request is for.

  8. In the FIS account box, type the FIS account number or click the magnifiying glass to search for the account and then click the FIS account name to select it.
  9. In the Departmental Account box, type the Departmental account or click the magnifiying glass to search for the departmental account and then click the departmental account name to select it.
  10. In the Phone Number box, type the phone number to be moved.
  11. In the From Building box, type the Building name or click the magnifiying glass to search for the building and then click the building name to select it.
  12. In the From Room Number box, type the Room Number.
  13. In the To Building box, type the Building name or click the magnifiying glass to search for the building and then click the building name to select it.
  14. In the To Room Number box, type the To Room Number or click the magnifiying glass to search for the room number.
  15. In the Notes/Comments box, type any desired notes.
  16. In the Order this item section, click Add to Cart button and then click the Continue Shopping button. When you are finished requesting items, click the Proceed to Checkout button.

    Note: An email will be sent to you confirming your order.

Change Telephone

  1. Go to https://kstate.service-now.com/.
  2. Under Self-Service, click Service Catalog.

  3. In the Telecom Work Orders box, click Telephone Work Orders.

  4. Click Change Telephone.
  5. In the On Site Contact (eID) box, type the eID of the person who is the on site contact or click the magnifiying glass to search for the contact, and then click the contact's name.

    Note: By deafult, the person filling out the request is displayed in the On Site Contact (eID) box.

  6. In the Contact Phone Number box, type in the contact phone number of the person you would like to be contacted in case there are questions about the request.
  7. In the Requested For User (eID) box, type the eID of the person you are requesting the service for or click the magnifiying glass to search for them and then click their name.

    Note: If this request is for someone who doesn't have an eID or if it is for a location that doesn't have an eID associated with it, then just put in either the on site contact or the person filling out the request. In the Notes/Comments box, describe who the request is for.

  8. In the FIS account box, type the FIS account number or click the magnifiying glass to search for the account and then click the FIS account name to select it.
  9. In the Departmental Account box, type the Departmental account or click the magnifiying glass to search for the departmental account and then click the departmental account name to select it.
  10. In the Phone Number box, type the telephone number to be changed.
  11. In the Display name box, type the user's name as they would like it to appear on caller ID.

    Notes:

    • The display name can be user name, department, location, etc.
    • The display name should be limited to the number of charcters that can be put into a phone switch.
  12. In the Phone Type section, click the type of phone you are requesting.

  13. Under Add Phone Options, select desired phone options to add.
  14. Under Remove Phone Options, select desired phone options to remove.
  15. In the Notes/Comments box, type any desired notes.
  16. In the Order this item section, click Add to Cart button and then click the Continue Shopping button. When you are finished requesting items, click the Proceed to Checkout button.

    Note: An email will be sent to you confirming your order.

Change Telephone Display Name

  1. Go to https://kstate.service-now.com/.
  2. Under Self-Service, click Service Catalog.

  3. In the Telecom Work Orders box, click Telephone Work Orders.

  4. Click Change Telephone Display Name.
  5. In the On Site Contact (eID) box, type the eID of the person who is the on site contact or click the magnifiying glass to search for the contact, and then click the contact's name.

    Note: By deafult, the person filling out the request is displayed in the On Site Contact (eID) box.

  6. In the Contact Phone Number box, type in the contact phone number of the person you would like to be contacted in case there are questions about the request.
  7. In the Requested For User (eID) box, type the eID of the person you are requesting the service for or click the magnifiying glass to search for them and then click their name.

  13. In the Reset voicemail password box, select if you would like the voicemail password to also be reset.

  15. In the Order this item section, click Add to Cart button and then click the Continue Shopping button. When you are finished requesting items, click the Proceed to Checkout button.

    Note: An email will be sent to you confirming your order.

Disconnect Telephone

  1. Go to https://kstate.service-now.com/.
  2. Under Self-Service, click Service Catalog.

  3. In the Telecom Work Orders box, click Telephone Work Orders.

  4. Click Disconnect Telephone.
  5. In the On Site Contact (eID) box, type the eID of the person who is the on site contact or click the magnifiying glass to search for the contact, and then click the contact's name.

    Note: By deafult, the person filling out the request is displayed in the On Site Contact (eID) box.

  6. In the Contact Phone Number box, type in the contact phone number of the person you would like to be contacted in case there are questions about the request.
  7. In the Requested For User (eID) box, type the eID of the person you are requesting the service for or click the magnifiying glass to search for them and then click their name.

    Note: If this request is for someone who doesn't have an eID or if it is for a location that doesn't have an eID associated with it, then just put in either the on site contact or the person filling out the request. In the Notes/Comments box, describe who the request is for.

  8. In the FIS account box, type the FIS account number or click the magnifiying glass to search for the account and then click the FIS account name to select it.
  9. In the Departmental Account box, type the Departmental account or click the magnifiying glass to search for the departmental account and then click the departmental account name to select it.
  10. In the Phone Number box, type the telephone number to be disconnected.
  11. In the Building box, type the Building name or click the magnifiying glass to search for the building and then click the building name to select it.
  12. In the Room Number box, type the Room Number.
  13. In the Phone Type section, click the type of phone you are requesting.
    • Analog - single line, desk phone
    • Analog Cordless - cordless, single line, desk phone
    • Wireless IP - multi-line phone, utilizes campus wireless IP infrastructure
    • Digital - multi-line desk phone
  14. Under Add Phone Options, select desired phone options to add.
  15. In the Notes/Comments box, type any desired notes.
  16. In the Order this item section, click Add to Cart button and then click the Continue Shopping button. When you are finished requesting items, click the Proceed to Checkout button.

    Note: An email will be sent to you confirming your order.

Multiple Telephone Work Order

If you have more than 10 items that you want to submit, you can use the Multiple Telephone Work Order. This option allows you to download a spreadsheet and put the details of the order in the spreadsheet. You will lose the individual item tracking capability when using this option.

  1. Go to https://kstate.service-now.com/.
  2. Under Self-Service, click Service Catalog.

  3. In the Telecom Work Orders box, click Telephone Work Orders.

  4. Click Multiple Telephone Work Order.
  5. Click the desired template.
  6. Complete and save the template and then click Click Here To Attach Your Completed Multiple Order Template button to upload the template.

    • Select 'Choose Files' from the attachments popup and navigate to the file you want to attach, select the File, and then click Open
    • Select 'Attach'
    • Choose the 'x' in the upper right to close the attachment popup

      Note: Attachments are required to submit multiple Work Orders

  7. In the Multiple Order Type list, select the type of work order you are submitting.
  8. In the Departmental Account box, type the Departmental account or click the magnifiying glass to search for the departmental account and then click the departmental account name to select it.
  9. In the Quantity list box, select the quanity you are ordering.
