How to submit a request

The submission of a change request to the Change Management Board is a very simple process.

  1. Technical staff conceive and plan a change.  If the change affects other units, they should be involved in the early planning.
  2. Download a change request plan template and fill it out.
  3. Technical reviews should be conducted within the unit, with the manager reviewing and approving the change plan.
  4. E-mail the completed change request to the Change Management Board at: cmb-l@k-state.edu
  5. The Change Management Board will do the following:
    • Post notification of the change on the Status of IT Services web site
    • Place the change request in the appropriate location in the CMB archive (password required)
    • Notify the community of the proposed change through the CMB agenda sent to: itproblems@k-state.edu
    • Review the change, notifying the department of the results
  6. The sponsoring unit receives and acts upon feedback from the CMB and colleagues
  7. Conflicts will be resolved utilizing the normal management structure
  8. Technical staff execute the change plan
    • Download the e-mail notification template and fill it out as a change announcement
    • Prior to starting the change, send the e-mail notification to itproblems@k-state.edu & other appropriate groups
    • Perform the IT service event as described in the change plan
    • After the change is completed, if any significant time lapse has occurred
      • Download the e-mail notification template and fill it out with information about the change completion
      • Send the e-mail notification to itproblems@k-state.edu & other appropriate groups
