The completed original form should be signed and forwarded to Position Administration,
Division of Human Resources
with a copy to the Office of
Affirmative Action in the recruitment packet.
Form Completion Instructions
Enter identifying information at the beginning of the form.
A position number (HRIS) is required before the position description can
be reviewed by
HR or
AA.
The job
code and the job title must match.
SECTION A. CONDITIONS OF APPOINTMENT
- Enter requested information.
SECTION B. ORGANIZATIONAL STRUCTURE
- Enter requested information (1 and 2).
- Write a brief summary of the role the position has in the unit.
SECTION C. DESCRIPTION OF JOB RESPONSIBILITIES
- Describe the duties of the position.
SECTION D. QUALIFICATIONS
- List education required.
- List experience required.
- List other requirements such as licenses or certifications.
- List preferred qualifications.
- Indicate a salary or range.
SECTION E: Attach an organizational chart
