New Hires/Rehires:

New Hires

• Hourly Student Data Sheet (PER-40) (pdf)
• Employment Eligibility Verification (Form I-9) (pdf) and supporting documents
• Social Security Card is required for all new hires (even if it is not used with the I-9)
• K-4 Form (pdf), Kansas Employee's Withholding Allowance Certificate
• W-4, Employee's Withholding Allowance Certificate (pdf) (Only if employee desires federal withholding other than single with 0 allowances or if employee does not update via self service) View/Update W-4 Tax Information
• State of Kansas Substance Abuse Form (pdf) required by the Substance Abuse Policy
• Intellectual Property Agreement (pdf)
• Direct Deposit form (pdf)
• Additional documents for Non-Resident Alien employees:
• F-1 Student
• J-1 Student
• J-2

Rehire

• Hourly Student Data Sheet (PER-40) (pdf)
• Employment Eligibility Verification (Form I-9) (pdf) is required if there has been a break of 30 days or more. In most cases Section 3 of form can be completed.
• On Campus Work Permit (if applicable)
