Workers' Compensation Changes — New and Improved!

No later than May 1, 2014, Human Resources is switching to a modified State of Kansas 1101-A form for reporting Accidents and Injuries. The PER 17 will be abolished, please delete any saved or cached copies on your computers.

Use of the 1101-A allows for the quickest possible submission to the State Self-Insurance Fund, and puts the needed documents into the hands of employees at the time of the accident or shortly thereafter. It also ensures that accurate information will be transmitted to the State. The form is an easy to complete on-line document. Benefits Administration asks that the form be attached to an email sent to, further speeding up the process. Notifying us via email or phone about an accident has been eliminated; however, the completed accident report is still due in HR within three working days of the accident date.

Completing the fillable form is standard data entry, with additional instructions and information for injured employees on page two. Please read those instructions carefully to become familiar with the documents you will need to provide the employee in the event of an accident.

We will still use "Accident Report" when referring to the 1101-A. You should begin using the form as soon as it is published, even if before May 1, 2014.

Human Resources Seminars in April!

Legacy Services will host a lunch and learn on the topic of Long-term Care Insurance. Two sessions will be held over lunch hour on April 17, 2014 at 11:30 am and 12:30 pm in Leadership Studies Building, Wayne Franklin Conference Room. Lunch will be provided. Three informational meetings on the topic of short-term disability insurance will be held on April 24 in the Wayne Franklin Conference Room of the Leadership Studies Building.
