USCIS Revises Employment Eligibility Verification Form I-9
U.S. Citizenship and Immigration Services (USCIS) has revised the Employment Eligibility Verification Form I-9.   The specific revisions include removal of five documents from List A which establishes both identity and employment eligibility.  The revised List A now includes: a U.S. passport (unexpired or expired); a Permanent Resident Card (Form I-551); an unexpired foreign passport with a temporary I-551 stamp; an unexpired Employment Authorization Document that contains a photograph (Form I-766, I-688, I-688A, or I-688B); and an unexpired foreign passport with an unexpired Arrival-Departure Record (Form I-94) for nonimmigrant aliens authorized to work for a specific employer.

As of November 7, 2007, the Form I-9 with a revision date of June 5, 2007 is the only version of the form that is valid for use.   Please begin using the new version immediately for newly hired employees and for employment reverification when: (1) an employee’s work authorization expires, or (2) an employee is rehired.  The new Form I-9 is currently available at http://www.k-state.edu/hr/forms.  In addition USCIS has also updated Publication M-274, "Handbook for Employees, Instructions for Completing Form I-9" at http://www.uscis.gov/files/nativedocuments/m-274.pdf .

2007 W-2 Wage and Tax Statement Address Update
The 2007 W-2 Wage and Tax Statements for KSU employees will be mailed directly to the employees’ home address by the Division of Accounts and Reports by January 31, 2008. The address that is currently being printed on the employee’s payroll warrant/advice mailer will be the address used to send the 2007 W-2 Wage and Tax Statement. Our office will mail local agency W-2 forms with postage being charged to the individual local agency department. Note: Employee’s with addresses not within the USA will have their W-2’s sent to our office and we will in turn mail them to the employee.

A memo dated November 13, 2007 was sent to Personnel Specialists along with “2007 W-2 Wage and Tax Statement Address Update for Inactive Employees” of any employee, who separated from the university from December 17, 2006 through December 15, 2007, including students. If a change is necessary, departments have until December 17, 2007 to update Home Addresses on line in HRIS, unless it is a foreign address. As a reminder when updating out of state addresses please use “ZZ” as the county code. For complete instructions on how to update addresses go to http://www.ksu.edu/hr/human.resource.units/hris_manuals.html. Changes to be updated by HR must be submitted by December 7, 2007.

NOTE: For inactive employees only, you may update home addresses at any time, including the 1st week of the pay period, as long as all employee records are inactive.

2008 Tax Treaty Renewal for Non Residents
Each year non resident employees who are eligible for tax treaty benefits must complete new forms, IRS Form 8233 and Appendix A (students) or Appendix B (professors, teachers) in order to receive the benefits.  On November 14, 2007 a memo was sent to Personnel Specialists along with a listing of non resident employees who had tax treaty benefits in 2007. You may view this memo at http://www.ksu.edu/hr/news/hrmem.htm. In order for employees, if applicable, to have tax treaty benefits begin with the first check they receive in January 2008, the new forms need to be returned to Human Resources by December 5, 2007. Workers’ Compensation – Mercy West Occupational Health
Mercy West Occupational Health is the contracted medical provider for Workers’ Compensation related non-life threatening illnesses or injuries occurring to employees working in the Manhattan area.  Mercy West Occupational Health has advised that their facility will be closed for the following holidays:

            Thanksgiving – November 22 and 23, 2007

            Christmas – December 24 and 25, 2007

            New Years Day – January 1, 2008

Mercy West Occupational Health will be open on December 31, 2007, as a normal business day.  The triage hotline will be answered 24/7.  That number is 785-323-6000.

Life threatening illnesses or injuries – call 911 or report directly to the Emergency Room at Mercy Regional Health Center, 1823 College Avenue.

November 30, 2007 Paycheck
The November 30, 2007 paycheck is the third paycheck in the month.  Therefore, premiums for group health insurance, KPERS optional group life insurance, TEA life insurance and contributions to the KanElect flexible spending accounts will not be deducted from this paycheck.  Additionally, 12-month unclassified employees do not earn an 8-hour vacation day for the pay period November 4, 2007 through November 17, 2007, paid November 30, 2007.  Also, unclassified employees appointed to less than 12-month non-instructional positions do not earn vacation leave for this same pay period.
