Human Resources sometimes receives tax clearance forms that do not accompany the other new hire paperwork. Occasionally we have difficulty reading the signature and cannot determine the name of the employee. Please assist us by printing the name of the employee at the top of the form. We greatly appreciate your help with this!

This report has been developed to track workers compensation adjustments. It provides a list of those with workers compensation adjustments by pay period within selected department/department group and displays pay period end date(s), workers compensation earnings code(s), total hours by earnings code(s) and grand totals. This report will normally be run after the final pay calculation for the pay period. Report request parameters allow the report to be run for multiple pay periods, if needed.

The flu is here. There have been a number of H1N1 or Influenza like Illness (ILI) reported in students as well as faculty and staff. Because of this, the Division of Public Safety is initiating data collection of flu incidents. The Office of Student Life is already compiling this data for students.
In this regard, all employees are reminded that if they have the flu, they should not come to work. They should stay at home, consult with their physician if they feel the need, and not return to work until their fever is below 100 F for 24 hours without the use of fever-reducing medications. It does not matter if it is H1N1 or some other influenza variant. If you have the flu properly report to your supervisor that you are home sick with the flu. Supervisors are asked to report your incidence of flu to the department personnel specialist.
Department personnel specialists are required to complete the on-line flu survey when they report the hours worked and/or leave used in HRIS. The survey must be completed whether your department has employees out with the flu or not. All data compiled will be general and not connected personally to the employee. A link to the survey will be sent out at the end of each pay period for completion by department personnel specialists or those staff members who enter time and leave in your department.
