Revised KSU PPM Chapter 4950
The KSU Policy and Procedures Manual Chapter 4950, Clearance Procedures at Separation, has been changed effective July 19, 2002. The Employee Separation Checklist (PER 24) has been updated to include asking employees if they desire to complete an Authorization to Release Information form. The Authorization to Release Information form may be found in the KSU PPM Chapter 4010, Employment Verifications and References, and gives authorization for the employer to give specific information noted on the form, to prospective employers upon the employees request for an employment reference.

The location or link for the PER-24 has also been updated in the PPM chapter.

New Long Term Care Provider
The Kansas State Employees Health Care Commission (HCC) has approved MedAmerica as the new Long Term Care coverage provider for the Kansas State Employees Long Term Care Program. All employees and retirees of the State of Kansas, their spouses, children, siblings, parents, parents-in-law, grandparents and grandparents in law are eligible to apply for coverage. Formerly, this program was administered through a contract with Hartford Life Insurance Company. The HCC has also approved important plan design changes that continue to keep this program cost effective, up-to-date, and quality focused.

Since this is a provider and plan design change, the HCC is offering active employees a one-time open enrollment period for the Long Term Care Program, to be held from August 1 through September 30, 2002. Thereafter eligible employees can still apply for Long Term Care coverage using an expanded underwriting process.

To help better inform eligible and current program participants, MedAmerica is developing a web site to explain the new program. The web site will be available after July 18, 2002. You may access the web site at: www.ltc-kansas.com. MedAmerica may also be contacted at 1-877-212-6066 beginning August 1, 2002.

The KSU Division of Human Resources and the State of Kansas Division of Personnel Services are jointly hosting two Long Term Care seminars on August 13, 2002 - one at 10:00 a.m. and a duplicate at 1:30 p.m., both in K-State Student Union room 212. A representative from MedAmerica will conduct the one hour seminar, and outline the new Long Term Care plan provisions. To enroll in one of these seminars please RSVP on the web at: www.ltc-kansas.com.

Questions should be addressed to the KSU Division of Human Resources at (785) 532-6277.
