KSU PPM Chapter 3045 has been updated

The Official Bulletin Boards PPM Chapter 3045, has been revised effective May 6, 2013 to update the Family and Medical Leave Act and Workers Compensation (KWC-40) posters, and to add the Workers Compensation (KWC-40A) poster that applies to dates for accidents on or after April 25, 2013.

Please print these three posters and place or replace them on the Official Bulletin Boards in your area to ensure that Kansas State University is in compliance with federal laws. For questions, please call Kris Mauck at 785.532.6277.

New Form I-9 required after May 7, 2013

May 7, 2013 was the deadline for employers to discontinue using the old Employment Eligibility Verification Form I-9. Employers must begin using the new form that was released on March 8, 2013 for new hires and reverifications. Human Resources will no longer accept the old version of Form I-9.   • Log in to your account on the wellness portal at and click on 'Biometric Screenings' from the left menu and then select 'Onsite Screenings'
  • If you do not have internet access or have trouble with the online site, call toll-free 1-888-275-1205 and select option 5 for assistance making an appointment
  • The early slots fill up quickly so make your appointment as soon as possible
  • The appointment system closes two full business days in advance of an event
