Graduating hourly students may work up until classes begin in the fall — August 17th, 2012. For graduate students who are graduating, they must be moved to an hourly student appointment for the summer if their employment is continuing.

The forms for Shared Leave requests and donations have been updated, expanded, and assigned a PER number each, PER-64A&B for request and medical documentation and PER-64D for donations. The forms were redone to better capture the information needed from both the employee and the health care provider. Contact Stephanie Harvey or Elizabeth Mowry Harbstreit if you have questions.
Employees have the ability to identify and/or modify emergency contact information via HRIS Employee Self Service. Please encourage employees to complete this important information to ensure that the department or HR may promptly respond to a medical emergency.
Department personnel specialists may also identify and/or modify an employee's emergency contacts using the Emergency Contact pages. Updating this information in either component (Employee Self Service or Emergency Contact page) will immediately update the other component after saving the data. The information the employee provides is strictly confidential, to be used ONLY in an emergency.
