GTA/GRA Health Insurance – Summer 2006 Semester
As you hire or reappoint graduate teaching assistants and/or graduate research assistants for the Summer 2006 semester please advise each student about the available GTA/GRA health insurance plan.  Persons appointed at least .5 FTE for the summer as a graduate teaching assistant or graduate research assistant are eligible to participate in the health insurance plan.  The coverage period will be June 1, 2006 through July 31, 2006.  The open enrollment period will be June 1, 2006 through June 30, 2006.

A brochure describing the plan provisions and an enrollment form (including premium rates) are available from the Division of Human Resources web site at: www.ksu.edu/hr/benefits/gta.html, or at 103 Edwards Hall.

NOTE:  Please remind anyone enrolling for the GTA/GRA health insurance plan that there will be a slight delay in transmitting the premium payment and enrollment form to Student Resources inasmuch as the Division of Human Resources is required to verify their appointment and participation eligibility prior to sending the documents to Student Resources.

Anyone currently enrolled in the GTA/GRA health insurance plan and who are NOT reappointed to a .5 FTE or greater GTA/GRA position during the Summer 2006 semester, may continue with the health insurance plan by completing a Continuation Insurance Enrollment Card and submitting it directly to Student Resources.  Please note there is NO employer contribution for this type of enrollment.  The deadline for submission of the Continuation Insurance Enrollment Card is June 1, 2006 to Student Resources.  The Continuation Insurance Enrollment Card is available from the KSU Division of Human Resources, 103 Edwards Hall.

Questions concerning this plan and its provisions should be directed to Student Resources Kansas Customer Service at 1-888-344-6104.
