eID Affiliation for Student Employees
All students have a K-State eID.  When a student becomes an employee, they do not need to create a new eID.  Instead, they simply affiliate as an employee after being hired in HRIS.  Instructions for K-State eID Affiliation Process for New Employees:  http://www.ksu.edu/hr/hrisuser.htm.  This affiliation process provides access to HRIS Employee Self Service information.

Note: If a student with a license plate eID (initials and then 4 numbers) becomes a full time employee, they may request a one-time free eID change by completing the eID Change Form. This form may be requested and/or completed in the IT Help Desk office located in 313 Hale Library.

President’s Scholarship Deadline Approaches
The application deadline for the 2005-2006 president’s scholarship for classified employees is April 15, 2005.  This scholarship is a one academic year scholarship awarded to an outstanding classified employee taking classes on a full-time basis or to multiple classified employees taking classes on a part-time basis for completion of an undergraduate or graduate degree at KSU.  Further scholarship information and an application form may be obtained from the Division of Human Resources at (785) 532-6277 or by visiting the Benefits Administration web page at: http://www.ksu.edu/hr/benefits.

Prescription Plan Information
Caremark is the prescription plan provider for employees and dependents enrolled in any of the State of Kansas health insurance plans.  Recently Caremark mailed informational documents to the enrolled employee’s home address, consisting of a plan description brochure, a prescription drug claim and mail order form, a list of major chain participating pharmacies, and a drug formulary listing.

These documents were provided in compliance with the plan contract with the State of Kansas, for information purposes only.

Verification of Work Phone and Work Address in HRIS
Verification of Work Phone and Work Address in HRIS can be done by accessing the “Active Position Listing” report.  This report provides information about active positions work address/phone, employee job title and/or business title as well as reports to number, title and name.  In the past we have usually sent this report out at the time we are gathering information for the Campus Phonebook.  The report is now available for departments to run in HRIS.  Instructions are available at http://www.ksu.edu/hr/hris/hris-reports.html under “Other Reports.”

Even though the Campus Phonebook is updated once a year information is provided nightly to Computing and Network Services to update the K-State People Directory (online university directory) which also lists information regarding employees’ work phone, campus address and title.  In an effort to improve data accuracy of the online directory, we are asking that you run the report and verify that the work address/phone and employee job title and/or business title is correct.  If you find any discrepancies please cross out the incorrect information and indicate the new information and return a copy of the listing to Human Resources as soon as possible but no later than April 8, 2005.   Please refer to the memo dated March 11 outlining the information we would like you to review at http://www.ksu.edu/hr/news/hrmem.htm.
