For security purposes email correspondence or attachments should not contain an employees' personal identifiable information such as their social security number. Certain HR documents require this number to be listed on the form. These documents should be submitted to HR via campus mail, hand delivered to our office in Edwards Hall, or in those instances where there is some urgency, the form may be sent to HR via fax. Please contact your HR liaison with questions at (785) 532-6277.
This report provides a history of leave earned, used, and adjusted for
each benefit eligible employee within a designated department. It is an
ideal report to distribute to employees once or twice a year. From Date
and Through Date are defined within the report request parameters as
well as pay group and subunit. Twenty pay periods of leave history may
be selected to appear on a single page for each employee.
