Completing Employment Eligibility Verification (Form I-9
The Employment Eligibility Verification (Form I-9) must be completed for all new employees and copies of documents used for verification must be submitted with their appointment forms. The employee completes Section 1 on the Form I-9, and the employer completes Section 2. The Form I-9 must be signed within three (3) business days of the appointment date or earlier. When updating and reverifying an employee’s work authorization a Form I-9 must also be submitted. For complete instructions and a list of supporting documents access or contact your Information Management liaison at (785) 532-6277.

In addition, listed below are key points that should be considered when completing the form:

Under Section 1:

If a lawful permanent resident or alien authorized to work is checked, the alien number or admission number must be indicated. This number is usually found on the I-94. NOTE: If Section 1 is typed or completed by someone other than the employee the "Preparer and/or Translator Certification" area must be completed.

Under Section 2:

Documents from List A establish both the identity and the employment eligibility. When an unexpired foreign passport with the Form I-94 is used, list the information from the passport first then list the number and expiration from the I-94. The expiration date from the I-94 is usually D/S for duration of stay. NOTE: For documents that establish employment eligibility (list C) a social security card that states "Valid for work only with INS Authorization" is not a valid document for I-9 purposes.

Under Section 3:

When an employee’s work authorization expires, you must reverify his or her employment eligibility. Please indicate the employee’s name in Section 1. The employee must present a document that shows either an extension of the employees initial employment authorization or new work authorization. This includes a new on-campus work permit.
